Role Description
This is a full-time, on-site Case Manager/ Nurse role based in Kampala. The Case Manager/ Nurse will oversee care plans, coordinate services, conduct assessments, and ensure clients' needs are met effectively. They will maintain close communication with clients and families, provide guidance for caregivers, and monitor the quality of care delivered. The Case Manager/ Nurse will also advocate for client welfare, collaborate with healthcare professionals, and maintain accurate records of all client-related interactions.
Qualifications
- A minimum of a Diploma in Nursing
- Prior experience in elder care or a related healthcare field is beneficial
- Experience in client needs assessment, care planning, and coordination of services
- Skills in clear communication, client advocacy, and relationship management
- Organizational abilities for maintaining accurate records and managing multiple tasks
- Empathy, problem-solving skills, and a proactive approach to addressing client concerns
